Applications will be received till July 23rd 2008.

The interview will be held at 9:00 a.m. on 01st August 2008. Candidates are advised to report to the Director's Office.

I. Method of Application:
            Application form and Prospectus can be obtained from the Honorary Director’s Office on a nonrefundable payment of Rs.200/- (Rupees two hundred only) by cash or a crossed DD payable to C.S.I. Hospital, Bangalore – 560 051. 

Please Note:

1. Applications must be submitted in the prescribed form.All entries must be made by the candidate, legibly, in his/her own handwriting.
2. Completed application form must be sent to the Honorary Director, CSI Hospital, Bangalore. 
3. Dates of applications and interviews will be informed.  “Early Bird” applications are encouraged. The last date for issuing application forms and prospectus is 16th June 2008 and last date to receive completed / filled applications is 23rd June 2008.
4. The application must be submitted with a registration fee of Rs.150/- (Rupees One hundred and fifty only) by a crossed demand draft drawn in favor of C.S.I. Hospital, Bangalore, or Cash at CSI Hospital Counter following  all the Instructions given in the Application Form.

5. Copies of the following, as instructed, must be included.    
1. S.S.L.C. Mark sheet – 2 copies
2. Twelfth Standard/Pre-University/ Pre-Degree/Intermediate Marks sheet - 2 copies
3. Certificate of higher qualification / Degree / Training /    Computer etc., if any
4. Transfer Certificate from the School/College last attended
5. Medical Fitness Certificate
6. A Character and Conduct Certificate from the Head of the Church and for non-Christians, a Conduct Certificate from a responsible citizen who has known the candidate for a minimum period of two years immediately prior to seeking admission.
7. Stamped self-addressed Envelope, 9” x 4”

Note:  Candidates awaiting results of the qualifying examination are also eligible to apply, provided they produce the original marks sheet/pass certificate at the time interview for selection.

INCOMPLETE APPLICATIONS WILL BE REJECTED.
           
            All candidates called for interview will be informed by post Under Certificate of Posting.  The Institution is not responsible for the non-receipt of the Interview Card by post.

            The courses start in 1st of August each year. 

            Selected candidates should report to the Hospital on the date specified on the Selection List with their original certificates and 10 passport sized photographs.
