DEPARTMENT OF ENT

Obstructive Sleep Apnoea Syndrome

Snoring is a commonly heard sound in the night when the whole world is asleep. For some of us it is a disturbance,  for some it is fun to hear and laugh but for the one who is snoring, little do they know that it is harmful to them. This is due to a medical condition called Apnoea or loss of breath, which lasts for about 10 seconds or longer, occurring more than 5 times during an hour of deep sleep.  Obstructive Sleep Apnoea (OSA) occurs due to the narrowing of the air passage between the soft palate and upper end of the epiglottis. Complete narrowing causes apnoea ( loss of breath) and partial obstruction causes snoring. This is due to obesity secondary to excess intake of  fatty oily food, alcohol and  lack of physical exercise, all of which relate to the present day life style. It is the most common cause in 1% of normal population and 5% of the asymptomatic  population. In children it is due to Hypertrophied Adenoids and less commonly Mandibular  Abnormalities.

Symptoms:  More than 60% present with loud snoring, excessive sleepiness in the daytime, feeling of choking, shortness of breath, restlessness, drowsiness, frequent change in posture while at sleep and frequent passing of urine at night. Less common in some, who have morning headaches, apnoec spells and night sweats

Consequence of OSA:  Irresistible sleep while talking, driving, eating, feeling of sleepiness even after a good night sleep.

Investigations:  Visual observation -  frustrated struggle during inspiration(breathing in) often associated with noisy breathing with variable arousals. Pulse oximetry - monitoring of oxygen saturation while at sleep. Polysomnography - This is a gold standard test wherein the EEG, Muscle movements, Eye movements, Leg movements, Oxygen Saturation, ECG, Heart rate, Respirator rate, Blood pressure and Pulse are measured in an over night sleep.

Treatment of OSA: Avoidance of alcohol, sedatives and fatty food. Weight reduction exercises and mandibular advancement devices could be used. Continuous Positive Airway Pressure (CPAP) is used by a fan pump, which is delivered through a nasal mask. This is a pneumatic pump, which literally blows open the pharynx to allow unobstructed breathing. The surgical management is Uvelovelopharyngoplasty (UVPP) with the help of cautery or laser. The latest method is fibrosing the soft palate with Radio Frequency Probe at different settings. The other surgeries are Mandible Advancement Surgery wherein an airway is created in the pharynx.

The Bible tells of David singing “God gives His beloved good sleep” The thought is how many of us enjoy the “good sleep” or have a disturbed sleep due to noisy breathing – snoring.  This is an easily solvable problem with medical and surgical intervention, which is easily available.
